UrthWurm is a DOS clone of a popular game called SFCave. You control a "wurm" that moves forward through a cave-like environment with irregularly-shaped floor and ceiling, and occasional obstacles. At the start of a level, the wurm is positioned in the centre relatively to the floor and ceiling of the cave, but will gravitate towards the floor unless the player presses the space bar, which will move the wurm up a bit.

To keep the moving wurm more or less level you need to tap the space bar with a certain frequency, but as the shape of the cave changes and obstacles appear, it is required to move the wurm lower or higher depending on the situation.

There are 10 caves and three difficulty settings in the game. The colour of the wurm changes depending on difficulty, and the game tracks the highest score achieved for each difficulty.
